The Household/Neighborhood LPN delivers, coordinates, evaluates, and is accountable for the total nursing services for household/neighborhood residents during assigned shift. The Household/Neighborhood LPN performs nursing functions that require at a minimum a licensed practical nurse, including medication administration, observing residents' condition, and communicating with the Household/Neighborhood Clinical Leader or Physician, and treatments; and takes part in household/neighborhood activities with and for residents. Work is performed according to the community's policies and procedures within the scope of practice of the Virginia Board of Nursing Guidelines and in a safe and efficient manner. EDUCATION, EXPERIENCE, CERTIFICATION AND LICENSURE REQUIREMENTS * Graduate of an accredited school of nursing program with a diploma or a degree. * Licensed Practical Nurse in the state of Virginia; maintains license in active status. * Current CPR/AED certificate of completion. * ServeSafe training. Job Duties * Plans, oversees, and reviews services delivery and completion of nursing duties by LPNs and CNAs. Assists team members with resident services and activities of daily living as necessary to ensure quality services; instructs team members on correct procedures. Makes shift rounds with physicians. Follows proper infection control practices. Assesses resident injuries and renders first aid or refers resident for physician and/or emergency room treatment; completes incident report. Responds to resident call light requests promptly. May respond to emergency calls in Independent Living. * Receives report from off-going shift and gives report to oncoming shift. Communicates about resident condition with other team members; communicates changes in resident condition to the physician, guardian, and family. Notifies clinical leader of major problems and recurrent grievances. Keeps dining team informed of diet changes. * Within licensure, performs resident assessments upon admission; completes documentation for assessments, treatments, reactions, and outcomes; notifies appropriate resources (e.g., dietitian, physical therapist, etc.) of the need for evaluation; initiates the development of care plans (and MDS in Healthcare) for household/neighborhood residents; evaluates resident response to plan of care and recommends additional modifications as indicated. * Administers medications and treatments as prescribed by physician and nursing orders, according to regulations and established procedures. * Performs clinical rounds to ensure resident safety, comfort, and regulatory compliance; evaluates resident physical and emotional status. Educates residents (and others) about medical issues and healthy lifestyle choices. * resident clinical records according to established policies and procedures. * Ensures the clinical operations in the household/neighborhood comply with federal, state, and local regulations and the Nurse Practice Act. * Attends in-service education programs, team conferences, and nursing services conferences as scheduled; completes in-service programs as assigned. * Trains other clinical team members in nursing skills, and all team members in best practices. Serves as a role model of professional nursing practices. Advocates for quality clinical services. * All other duties as assigned. Qualifications * Knowledge of practices and procedures of long-term care; practices and procedures for clinical documentation. * Clinical skills sufficient to perform all essential job duties. * Ability to meet the needs of geriatric residents using patience, tact, and a desire to service those who are ill or incapacitated. * Ability to read and write, follow instructions, and communicate effectively in English. * Ability to assign, direct, and review the work of others. * Computer knowledge and skills with an aptitude to learn company software. PHYSICAL REQUIREMENTS AND WORKING CONDITIONS * Capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching, kneeling, reaching over the head, grasping with both hands, and fine manipulation. * Physically able to carry, transfer, push and pull, and reposition residents with assistance or using appropriate equipment. * Physically be able to lift up to 50lbs unassisted. * Normal eyesight or use corrective lenses to achieve it. * Ability to hear and distinguish between normal tones and be able to perceive the nature of sounds.
